Help with ESA50 Form

Hi, I wonder if anyone who has fairly recently successfully claimed ESA for Fibro would be able to help me fill in my ESA50 form? I need someone who is able to get by without help.

First questions are general and they are
1) Did you provide any medical reports with your ESA50 form or did you just leave it for ATOS to contact your doctor
2) Did ATOS contact both your GP and the Rheumatologist/other specialist for reports.

The other questions I have are about what you put into the form about your condition and abilities.

Your best bet to get help is see councils benefit advice department if they have one or else contact https://www.facebook.com/FightBack4Justice?fref=ts They are very good at what the do regards ESA and other benefits.

Mine was filled in by a council benefits advice worker who did filled form as fibro the day i went was bad, i got my esa and put in support group

Citizens Advice would help you fill out forms , they have trained counsellors who come in on certain days who can help with all kinds of forms :-D good luck !! x

I would strongly suggest you put in a much evidence/paperwork as you can. This shortens the process as contacting GP/hospitals takes alot of time.

As far as im aware no, they didnt contact anyone. I did tho put in alot of paperwork.

You put down as much info as you can about how your illnesses and conditions effect your daily life. If anyone has to help you do any tasks and activities or if you do these alone how much pain and exhaustion you in.

CAB did my forms.

When someone asks for help with completing a benefit claim form, the usual advice is, if you haven't already tried it, to seek help from a local support organisation such as CAB, Welfare Rights etc. As well as advice, they may also be able to help with actually completing them.

If you're unable to get to such peoples, there are many, many guides now available online that might be helpful. Admittedly, it does take time to go thru various sites, guides etc, but it is possible to piece things together yourself.

Benefits and Work might be a good place to start, altho to access the entire site, a paid membership is necessary. If you can break your conditions down into smaller components, such as pain, fatigue etc, and ask Mr Googley about it in relation to completing your ESA50, references should come up which, altho not related to FM, could give you some useful bits that you can use.

There isn't anyone on here/thru here that I'm aware of who provides such advice/help, which is why places such as CAB are suggested.

Hi folks, thanks for the replies.

Sorry, I should have made it clear that I am really looking for examples of what people have actually put on their forms rather than getting "advice". I want to see if I have missed anything or if there is a better way to describe things.
