Hi @ all

I have been enduring fibro for many years now, however, until recently it could manage it pretty well.
Since the end of 2012, things have taken a rough turn. I have been off work a few times and the pain, especially in the morning, is sickening.
My GP tried me on Duloxetine - NEVER again, I suffer from chronic depression since my teens and Duloxetine almost pushed me into self harm (or worse). Didn't do anything for my pain either. Back on Fluoxetine now. For years I've been on Codeine 30mg which I take with either Ibuprofen or Paracetamol, recently I cannot manage my pain with that any more, even on max daily dose. For "emergencies" only, my GP has prescribed me a small amount of Tramadol 50mg, which I treat like gold dust, I am properly rationing them. I don't think my doc understands the severity of my pain.
I cannot take Gabapentin or Pregabline as they make me nauseous and sick, doc wants me to take Amitriptyllin, but I feel drained and exhausted enough without taking that.
I really don't know what to do next and how to get adequate pain relief. How did you get the message through to your GP?

How about asking to be seen at the pain clinic? I had a similar problem, the pain consultant told my GP to prescribe me as much Tramadol as I needed. I am "allowed" to take upto 400mgs a day, I have slow release tabs, I take one every morning = 100mgs and 2 every evening = 200mgs, I then take one in the middle of the day if my pain is extremely bad. I can't see why your GP is unwilling to treat your pain, unless he is worried about how it interacts with other medication or how it will impact on other conditions you have x

Thank for your reply, Lolo.

The only other medication I am on is Lisinopril for (borderline) high blood pressure.
Next time I see her I will mention the pain clinic and if I could have a referral. I want to continue to work as long as I can, but the way things are going at the moment I am getting rather hopeless. So far, her favourite solution is trying to get me to take Amitriptylline, but I really do not want to take them. I guess a pain specialist would be a positive step forward. Seems generally to be a last resort to try out the pain clinic for most of us. However, if your doctor is not willing to prescribe anything else it may well be worth trying that. However assuming you have a rheumatoid consultant you could also ask them via a phone call if they can recommend anything themselves or via your GP ?
Personally I found it difficult to be prescribed anything for the first year other than codiene or tramadol and eventually i was on repeat for morphine. So being rationed for tramadol seems completely ridiculous in my opinion.
Of course every GP works in many different ways so maybe you could try to see another in the same practice?

I also take Lisinopril as I had a stroke that escalated my Fibromyalgia 3yrs ago and cannot take codeine as I found it bunged me up I take Paracetamol,Amitripylene,Morphine when needed and would recommend that you persevere with Amitripylene as it is also an anti depressant,start on a low dose and work up to what is right,it does take a few weeks to get used to but now if I forget to take it I find pain is a lot worse.Would not reccomend Pain Management as I found that they were only interested in getting me to accept boundaries of Fibromyalgia which I already knew.

Amitriptyline can help with the sleep and pain aspects of FM, but doesn't suit everyone. The only way you'll know if it will help you tho is to give it a try. It needs about a month to get into your system and take effect, and can come with side effects, but these tend to start easing off after a while.

A referral to the Pain Clinic may be helpful as it could give you access to other meds, treatments etc. Tramadol is quite often used in trying to manage FM, so I'm also a little :-? :-? :-? :-? about why you have to ration them, unless it is because due to possible problems with other meds and/or conditions.

Seeing another GP might be something to consider, a fresh perspective on your situation and problems could mean you are able to find a more effective way to control your pain and symptoms, which would make a difference to things for you.


If your dog doesn't like someone, then you probably shouldn't either
